What our nails can tell us?

Nails, Claws and Paws

The nail is one of the structures in the body where internal physiology becomes visible to the naked eye. To a trained observer, nails can suggest something about our nutrition, heart, lungs, liver, iron stores, or even a serious illness we endured months ago.

Time is a major casualty in modern medicine: time to spend talking with our patients and performing a careful physical examination. We rely increasingly on tests and imaging studies and less on the evidence that is right in front of us. As a professor of mine would say sarcastically to students in class, “If all else fails, examine the patient.”

In the nineteenth century, before laboratory diagnostics existed in any meaningful form, physicians had little choice but to cultivate extraordinary powers of observation. Sir William Osler, who built Johns Hopkins Medical School into a model of clinical education, formalized the idea that peripheral findings in the nails, hands, feet, eyes, and skin were extensions of internal disease. That conceptual link became embedded in medical education through Osler’s influence. The philosophy was simple: observe first, test second. Nails were part of that visual literacy, taught as carefully as the use of a stethoscope.

Evolution of Nails

Nails are made of keratin, a tough fibrous protein also found in hair and the outer layer of skin. They grow from the nail matrix, the living growth center just behind the visible pale crescent at the base of the nail, the lunula. The nail plate itself is translucent; its healthy pink color comes from the blood vessels in the nail bed beneath it. Fingernails grow up to three millimeters per month, and toenails grow about one millimeter per month. This means it can take about six months to regrow a fingernail and up to 18 months to regrow a toenail.

Human nails are flattened versions of claws. In other mammals, the same keratinized structure forms talons for gripping or hooves for bearing weight. As primates evolved toward precision rather than predation, claws flattened. The nail became less a weapon and more a stabilizer and precise tool, protecting the fingertip, enhancing tactile feedback, and enabling fine motor control. Anyone who has broken or lost a nail can attest to this.

Names of diseases of the nail often begin with or contain the prefix onycho-, which comes from the Greek onyx, meaning nail or claw, a reminder of their evolutionary relationship to claws and talons.

The Nail as a Diagnostic Window and Archive

A healthy nail is smooth, gently curved, firmly attached, and pale pink.

Nails are built from nutrients delivered through the bloodstream and shaped by cellular processes that govern organ function. When illness, deficiency, inflammation, or metabolic stress disrupts those processes, the disturbance becomes encoded into the nail as it forms. Because nails grow outward over time, they preserve a chronological record of physiologic stress. They are, in effect, a slow biological archive.

A History of Looking

The diagnostic use of nails is not new. Hippocrates described digital clubbing, swelling of the fingertips and a change in nail angle, in patients with chronic lung disease in the fifth century BCE. For centuries it was known as “Hippocratic fingers,” one of the oldest recorded physical signs in medicine, recognized long before its mechanism was understood.

Let’s Look at the Nails

• Vertical ridges are common with aging and usually harmless.

• Horizontal ridges (Beau’s lines) reflect temporary interruption of nail growth after significant systemic stress such as severe infection, surgery, or chemotherapy. Because fingernails grow roughly three millimeters per month, the distance of the groove from the cuticle can help estimate when the event occurred. When multiple nails show the same line, it suggests a shared physiologic insult.

• Small white spots (leukonychia) are usually due to minor trauma to the matrix, not vitamin deficiency.

• Spoon-shaped nails (koilonychia) can signal iron deficiency.

• Pitting, or small depressions in the nail surface, is often indicative of psoriasis.

• Painless separation of the nail from the bed (onycholysis) may follow trauma, inflammatory disease, thyroid dysfunction, or chemical exposure.

• Infection of the sides of the nail (paronychia) is often bacterial but can also be viral or fungal.

Serious Nail Findings

• Clubbing, bulbous fingertips with exaggerated curvature of the nails, is associated with serious lung, heart, or liver disease and reflects chronic low-oxygen states.

• Splinter hemorrhages, thin red-brown streaks beneath the nail, can indicate infection of the heart valves.

• Pigment in the nail (melanonychia) may represent subungual melanoma. It is frequently mistaken for a bruise or fungal infection. This warrants urgent evaluation.

Fungal Nail Infection Deserves Special Mention and Effective Treatment Is Available

About 20% of older adults suffer from fungal nail infection. Much nail damage is mechanical and results from microtrauma. Tight shoes compress toenails, and repetitive impact drives them into the front of the shoe. Over time this can cause thickening, discoloration, or slight separation. That separation creates a warm, protected space beneath the nail plate. Add moisture and fungi thrive.

Diabetes, reduced circulation, immune compromise, and aging increase susceptibility.

Because psoriasis, trauma, and aging can mimic fungal infection, laboratory testing of the nail is recommended to confirm the diagnosis and identify the organism before treatment. Most cases are dermatophyte infections and respond best to oral terbinafine (Lamisil). Liver blood tests are indicated during treatment, which may continue for several months. If nail culture or testing identifies a different fungus, other medications may be required. The modern treatment approach is specific and organism-based.

Manicures and Pedicures

Nails are adornment for many of us. Many people get manicures and pedicures. But if you notice an abnormal nail or abnormal pigmentation, seek medical attention instead of covering the problem with gel, polish, or acrylic overlays.

Rigid gel treatments increase stress on a lifting nail and nail bed. Covering fungal infection traps moisture. Aggressive filing stresses the matrix. Trimming cuticles can cause infection.

When to Seek Evaluation

• New clubbing

• Sudden color changes or ridges across multiple nails

• Unexplained nail lifting

• Multiple splinter hemorrhages without trauma

• A new dark longitudinal band

Hard as Nails: Principles to Maintain Healthy Nails

Healthy nails are quiet: smooth growth, firm attachment, steady replacement. If a nail looks unhealthy, the first step is accurate diagnosis, reduction of stress on the nail, and recovery time.

• Wear wide toe-box shoes.

• Use cotton-free socks, such as wool socks, which wick moisture away.

• Control moisture by sprinkling cornstarch-based powder between the toes.

• Protect the cuticle and matrix by avoiding cutting nails too short or trimming cuticles.

• Ensure adequate protein intake (30–40 grams per meal, three times a day).

• Be patient. Fingernails can take six months to replace and toenails up to 18 months.

The nail is small. But it carries history: evolutionary, clinical, and personal.

It conveys important information to the trained eye.

Proof of Life: Work along with Play

Went to the Oregon coast this weekend, where I saw a “moon dog” for the first time in Arch Cape. I didn’t have my phone with me to take a picture but I found this image and text online. What I saw was sharper and even more impressive.

“A moon dog, moondog, or mock moon, (scientific name paraselene, meaning "beside the moon") is a relatively rare bright circular spot on a lunar halo caused by the refraction of moonlight by hexagonal-plate-shaped ice crystals in cirrus or cirrostratus clouds.

Moon dogs appear as part of the 22° halo, roughly 10 Moon diameters outside the Moon. They are exactly analogous to sun dogs, but are rarer because the Moon must be bright, about quarter moon or more, for the moon dogs to be observed. Moon dogs show little color to the unaided human eye because their light is not bright enough to activate the cone cells.”
